"304 Park Avenue S. NYC" address denotes [l156370] reissue from circa 2000s/2010s. This version has a facsimile cover of the 1967 mono release, but the recording is stereo. The typography has been mostly recreated with digital fonts that are a close but not perfect match to the original (e.g. the "C" in "Action" has slanting terminals whereas the originals are horizontal; the catalog number in the oval shape on the front has been horizontally stretched to match the width of the original, and next to it the "OT" letters in "BLUE NOTE" are closer together than the 1960s original).
